Useful Information

Everything you need to know before your visit, from patient forms to payment policies.

Diabetes Patients: Please bring your glucometer to all appointments.

Appointments

Patients are seen by appointment only. Please arrive 15 minutes prior to your scheduled time.

Have your co-pay and insurance card(s) ready for the receptionist. You will receive automated reminders via text, email, or phone 7 days and 3 days before your visit.

Please call to reschedule as soon as possible should you need to change your appointment.

Payment & Fees

Co-payments are due at time of service. If you are a self-pay patient, you will be charged a flat fee which is approximately 50-70% of what is customary.

Payment is expected in full at the time of service unless other arrangements are made.

Laboratory Services

In-house lab services are provided by Labcorp by appointment only. Note that lab work is billed separately by Labcorp.

You maintain the right to use any laboratory of your choice or have work done through your primary physician.

New Patients

A referral is required for new patient appointments. Please try to arrive 30 minutes prior to your appointment.

Your physician can call or fax a referral to our clinic. The referral should include your demographic and insurance information, recent lab work, your last office visit note and any other relevant tests.
